EVALUATION FORMS

GENERAL REFERENCE

  • Approved Clock/Credit Hour Providers — ALL clock hours and credits must be provided by an OSPI Approved Provider. The Provider list changes annually and may not include the same providers from one year to the next, please check the approved list to ensure your hours/credits will be counted for salary placement. The placement is based on credits and hours completed and submitted PRIOR to October 1st of each year.

  • Change of Address/Phone/Name — Please note: Primary phone number will be the number to get autodialer notifications.

  • Leave of Absence Request Form — Request for leave of absence/purpose of leave. Please note, PRIOR approval is required for personal leave by your supervisor/building principal.

  • Vacation Request Form 5400 F2 — Classified Staff Vacation Request. As indicated in Section 8.2.1 "Full-time employees must provide 5 days advance notice to their supervisor by submitting a vacation leave request form."

  • Transfer Request SCEA 4.10.5

  • Assignment Preference Request SCEA 4.10.3

  • Driver's Abstract — Complete and submit to HR for processing (Online abstracts are NOT acceptable unless pre-approved by HR to ensure correct format is requested.)

ACCIDENT REPORTING INFORMATION

  • All incident/accident claim reporting is now found online at www.pswct.org. Whether you choose to seek medical treatment for an injury or not, you are required to call the Nurse Triage Line at 833-928-6877 to submit an accident report.
